New Research Adds to What We Know About Kratom and 7-OH

The August 2026 systematic review adds an important new dimension to what researchers know about kratom, mitragynine and concentrated 7-OH exposure.

Historically, much of the public discussion surrounding kratom has focused on dependence, withdrawal, opioid-like effects, contamination and overdose. The new review draws additional attention to reported respiratory and pulmonary complications.

Researchers identified reports involving central respiratory depression, acute respiratory distress syndrome and other forms of acute lung injury, diffuse alveolar hemorrhage, hypersensitivity-related lung inflammation and pulmonary edema or congestion.

These findings are medically important, but they also need to be interpreted in context.

The review examined a relatively small collection of clinical and forensic reports. Researchers identified 24 reports overall, including 14 clinical reports and 10 forensic reports. The authors grouped the findings into provisional pulmonary categories to help clinicians and researchers recognize patterns that may warrant further investigation.

The findings do not establish that kratom or 7-OH directly caused every respiratory complication described in the underlying reports.

Pulmonary conditions such as ARDS, pulmonary edema and respiratory failure can have numerous causes. Polysubstance exposure can make causation even more difficult to determine because some people may have consumed additional substances capable of affecting breathing or consciousness.

For this reason, the researchers describe the proposed patterns as provisional and hypothesis-generating rather than definitive evidence of causation.

That distinction matters.

The responsible takeaway from the research is not that kratom inevitably damages the lungs. Instead, the findings suggest that clinicians and consumers should recognize respiratory complications as another potential area of concern that deserves additional research.

The findings are particularly relevant as concentrated 7-OH products have become more prominent.

Traditional kratom leaf naturally contains multiple alkaloids, including mitragynine and relatively small amounts of 7-hydroxymitragynine. Products specifically manufactured or marketed for concentrated 7-OH can create a substantially different exposure.

That difference has attracted increasing attention from regulators and public health officials.

For Kentucky residents, the distinction is especially important because the state has specifically addressed concentrated 7-OH through its controlled-substance regulations. Under 902 KAR 55:015, 7-hydroxymitragynine concentrated above 400 parts per million on a dry-weight basis is designated as a Schedule I controlled substance. The regulation explicitly states that this designation does not apply to mitragynine.

The regulatory distinction reinforces an important point for consumers: a traditional kratom product and a highly concentrated 7-OH product should not automatically be assumed to have the same composition, potency or risk profile.

Additional research will be needed to determine how frequently pulmonary complications occur, whether particular products or concentrations create greater risks, how dose influences respiratory effects and how polysubstance use contributes to reported medical emergencies.

In the meantime, severe sedation or breathing problems following kratom, 7-OH or other substance use should never be ignored.

Someone who cannot be awakened, has stopped breathing, is breathing very slowly or irregularly, develops blue or gray lips, experiences severe shortness of breath or loses consciousness needs immediate medical attention.

Call 911 immediately for a suspected overdose or serious breathing emergency.

For non-emergency situations, people who find themselves repeatedly using kratom or concentrated 7-OH despite wanting to stop may benefit from a professional substance use assessment.
